To track a GLP-1 course, log each weekly injection with its date, dose step and injection site, and follow the escalation your prescriber set. The Ozempic, Wegovy and Mounjaro labels all start at a low dose and step up no sooner than every 4 weeks.

This article quotes official labels. It is not a dosing guide: the dose, the pace of escalation and any change are decided by your prescribing doctor, and the leaflet in your own package is the authority for your product and country. This English article follows the US Prescribing Information (including Zepbound); the Ukrainian article covers the EU Summary of Product Characteristics (SmPC), so for EU wording read that version.

Which labels this article uses

Labels differ between regions. The tables below use the US Prescribing Information (PI) as the primary source and note where the EU Summary of Product Characteristics (SmPC) says something different. Zepbound is a US product, so it appears only in the US rows. All label facts were checked on 16 October 2026; labels are revised, so compare against the leaflet in your package.

Titration steps per label

Titration is the stepwise increase from a starting dose. The reason the labels give is quoted in the section on the first dose below.

Product (US PI)StartSteps as the label words themMaximum
Ozempic (semaglutide, type 2 diabetes)0.25 mg weekly for 4 weeks0.5 mg; “if additional glycemic control is needed after at least 4 weeks”, 1 mg, then 2 mg2 mg
Wegovy injection (semaglutide, weight management)0.25 mg, weeks 1–4Fixed 4-week steps: 0.5 mg (weeks 5–8), 1 mg (weeks 9–12), 1.7 mg (weeks 13–16), then maintenance from week 177.2 mg (Wegovy HD, adults)
Mounjaro (tirzepatide, type 2 diabetes)2.5 mg weekly”If additional glycemic control is needed, increase the dosage in 2.5 mg increments after at least 4 weeks on the current dose”15 mg (adults)
Zepbound (tirzepatide, weight and sleep apnea)2.5 mg for 4 weeks, then 5 mg”May be increased in 2.5 mg increments, after at least 4 weeks on the current dose”15 mg

Maintenance doses as the labels list them:

  • Ozempic, US PI: 0.5, 1 or 2 mg. For type 2 diabetes with chronic kidney disease, the US PI gives 1 mg as the maintenance dose. The EU SmPC lists no maintenance doses and says weekly doses higher than 2 mg are not recommended.
  • Wegovy, US: for weight reduction in adults “either 1.7 mg or 2.4 mg (recommended)”. For patients tolerating 2.4 mg for at least 4 weeks when additional weight reduction is clinically indicated, the dose “may be increased to a maximum dosage of 7.2 mg”, sold as a single-dose pen called Wegovy HD.
  • Wegovy, EU: Table 2 of the SmPC lists 2.4 mg and 7.2 mg as maintenance doses; 1.7 mg appears only as a titration step. The 7.2 mg dose can follow “a minimum of 4 weeks” on 2.4 mg in adults with BMI ≥ 30 kg/m² at treatment initiation.
  • Mounjaro, US: the PI (rev. 08/2026) gives the 2.5 mg start, 2.5 mg increments after at least 4 weeks on the current dose, and a maximum of 15 mg in adults (10 mg in pediatric patients); it lists no maintenance doses and no separate first step. The EU SmPC (section 4.2) does have one: “The starting dose of tirzepatide is 2.5 mg once weekly. After 4 weeks, the dose should be increased to 5 mg once weekly. If needed, dose increases can be made in 2.5 mg increments after a minimum of 4 weeks on the current dose.”
  • Mounjaro, EU, adults (SmPC): maintenance doses are 5, 10 and 15 mg, with a maximum of 15 mg (ages 10 and up with type 2 diabetes: 5 or 10 mg, maximum 10 mg).
  • Zepbound: 5, 10 or 15 mg for weight management; 10 or 15 mg for sleep apnea.

Where a label says “at least 4 weeks” (Ozempic from 0.5 mg upward, Mounjaro, Zepbound), that is a minimum interval, not a target; for the first step (Ozempic 0.25 mg, Zepbound 2.5 mg) the labels say “after 4 weeks”. The US Mounjaro PI words every increase as “after at least 4 weeks”; the EU Mounjaro SmPC words the first step as “After 4 weeks” and later increases as “a minimum of 4 weeks”. If a patient does not tolerate a dose, the US Wegovy PI says to “consider delaying dosage escalation for 4 weeks”; the EU SmPC says delaying escalation or lowering to the previous dose until symptoms improve “could be considered”. Whether and when to move up is the prescriber’s call.

Why the first dose is low

What the labels say:

  • US Ozempic, Wegovy, Mounjaro and Zepbound: the escalation schedule is there “to reduce the risk of gastrointestinal adverse reactions”.
  • EU Ozempic: “Semaglutide 0.25 mg is not a maintenance dose.”
  • Zepbound: “The 2.5 mg dosage is for treatment initiation and is not approved as a maintenance dosage.”

Missed dose and changing the injection day

All four products are once-weekly injections, at any time of day, with or without meals; the Ozempic and Wegovy US PIs add “on the same day each week”. The windows differ by product and region.

ProductMissed dose (per label)Changing the weekly day
Ozempic, US and EUAs soon as possible within 5 days after the missed dose; if more than 5 days have passed, skip itUS: at least 2 days (more than 48 hours) between doses. EU: at least 3 days (more than 72 hours)
Wegovy, USIf the next dose is “more than 2 days away”, administer as soon as possible; if less than 2 days away, skip itThe PI has no day-change rule; the Medication Guide says “you may do so as long as your last dose of WEGOVY was given 2 or more days before”
Wegovy, EUWithin 5 days after the missed doseAt least 3 days (more than 72 hours)
Mounjaro and Zepbound, USAs soon as possible within 4 days (96 hours); if more than 4 days have passed, skip itAt least 3 days (72 hours)
Mounjaro, EU4 days, same as the USAt least 3 days, same as the US

The US Wegovy PI adds that if 2 or more consecutive doses are missed, dosage escalation is reinitiated at a lower dosage; the EU SmPC says that if more doses are missed, reducing the starting dose for re-initiation should be considered. What to do after a missed dose in your own case is a question for your prescriber or pharmacist.

Injection sites and rotation

  • Ozempic and Wegovy: the labels name the abdomen, thigh or upper arm for the subcutaneous injection. The Ozempic PI says to use a different injection site each week when injecting in the same body region; the Wegovy PI says to rotate sites with each dose; the EU SmPCs say the injection site can be changed.
  • Mounjaro and Zepbound: inject under the skin of the abdomen or thigh, or have another person inject in the back of the upper arm. Both labels say to rotate injection sites with each dose.
  • Same area, different spot: the Wegovy instructions for use say you may inject in the same body area each week as long as it is not the spot used for the last injection. The Zepbound Medication Guide says you may use the same area but should choose a different site in it.
  • Never share a pen: the US labels say pens must not be shared between people, even if the needle is changed (the US warnings sections of the Ozempic, Wegovy and Mounjaro/Zepbound PIs).

Oral Wegovy is a separate product

The FDA approved a Wegovy tablet on 22 December 2025. It is a separate product with its own titration, timing and missed-dose rules, taken by mouth, and none of the injection rules above apply to it. Use the tablet sections of the Wegovy PI and its Medication Guide.

What to record at each injection

A GLP-1 course runs for months, so a short record helps you and your doctor see the pattern:

  1. Date and time of the injection.
  2. Dose step, as prescribed, with the pen or strength used.
  3. Injection site, so the next one is a different spot.
  4. Side effects you noticed, with the day they started.
  5. Questions for the doctor you want to raise at the next visit.
  6. Weight, only if your doctor asked you to track it.

HealthLab for tracking a GLP-1 course

HealthLab is an iPhone app for medications and lab results. A GLP-1 course in HealthLab is a ladder of doses in milligrams that you type, and you advance it step by step yourself. The app does not prescribe or auto-advance doses and does not tell you when to step up: the pace is whatever your doctor decided.

  • GLP-1 reference: a view-only screen with the EU SmPC titration steps for Mounjaro, Ozempic and Wegovy. It covers those three brands only.
  • Injection-site rotation: six sites (abdomen, thigh, upper arm, left and right), a body map and a hint for the next site. If your label says the upper arm should be injected by another person, follow the label.
  • Dose reminders are free, with lock-screen actions Taken and Snooze 15 min; skipping a dose is done in the app. A follow-up reminder, if a dose is not marked, is a Pro feature.
  • Journal, including side effects, is free. The weight chart on a course and the adherence report are Pro.
  • Courses with reminders, including GLP-1, are free on the main profile; additional profiles for family members come with Pro. Pro is a subscription with a 7-day free trial.

Data is stored locally on your iPhone via SwiftData, and no account is needed. The app is not a medical device and does not provide medical advice, diagnoses, or treatment recommendations. You can get HealthLab on the App Store. For the general course setup, see How to set up a medication course, schedule and reminders.

Frequently asked questions

What do I do if I miss a weekly GLP-1 injection?

The label windows are 5 days for Ozempic, 4 days for Mounjaro and Zepbound (96 hours in the US wording), and for Wegovy “more than 2 days away” from the next dose in the US versus 5 days in the EU; past the window, the label says to skip the dose. What applies to you depends on your product, region and situation, so check the leaflet in your package or ask your prescriber or pharmacist.

Where on the body can I inject Ozempic, Wegovy or Mounjaro?

Ozempic and Wegovy labels name the abdomen, thigh or upper arm. Mounjaro and Zepbound labels name the abdomen or thigh, or the back of the upper arm when another person injects. The US labels say to rotate sites; the Wegovy instructions and the Zepbound Medication Guide allow the same body area if it is a different spot.

Can I move to the next GLP-1 dose earlier than 4 weeks?

The Ozempic and Zepbound labels say “at least 4 weeks” on the current dose for later increases (their first step is “after 4 weeks”), the US Mounjaro PI says “at least 4 weeks” for every increase (the EU Mounjaro SmPC words its first step “After 4 weeks” and later ones “a minimum of 4 weeks”), and Wegovy’s table uses fixed 4-week steps. That is a minimum interval, not a target, and the decision to increase belongs to your prescriber.

Can I change my weekly injection day?

Labels allow it if the gap between two doses is long enough: at least 3 days for Mounjaro and Zepbound, at least 2 days for Ozempic in the US and 3 days in the EU, and 3 days for Wegovy in the EU; for Wegovy in the US the Medication Guide says you may change the day as long as the last dose was given 2 or more days before. Ask your prescriber before shifting the day.

Does HealthLab tell me when to increase my GLP-1 dose?

No. The ladder holds the doses you type and you advance it yourself; the app does not prescribe or auto-advance doses. The built-in reference only shows the EU SmPC titration steps for Mounjaro, Ozempic and Wegovy, view-only.
